California Schools At-Home Testing
Please use this secure platform to report your at-home COVID-19 test result to Lake County Public Health. Please report both negative and positive results to help track COVID-19 in our community.
If your school is already in Primary.Health, please use your school's results portal to submit your test result.
Exposure & Outbreak Reporting
School districts, schools, child care programs or extracurricular programs should fill out the Schools Portal for Outbreak Tracking (SPOT) Intake Form if the school setting or program has one or more confirmed COVID-19 cases.
To report cases and suspected outbreaks, complete the Schools Portal for Outbreak Tracking (SPOT) Intake Form. The SPOT Intake Form is also available in Spanish. SPOT Intake Form opens best with Edge or Chrome browsers.

California has launched the Safe Schools for All Hub to consolidate key resources and information related to COVID-19 and schools, including K-12, colleges and universities, as well as information for youth sports!
